Total length of your vehicle or setup from front bumper to rear (or tent/trailer end). Include any service windows or overhangs.
How wide your setup extends — including pop-outs, awnings, or service tables on either side.
Any extra clearance for customer lines, propane tanks, serving tables, or vehicle access that the coordinator should know about.

How to get your VDH Inspection Report:
  1. Visit vdh.virginia.gov — Food Safety.
  2. Call or email your local VDH district office (the one that issued your food permit) and request a copy of your most recent inspection report.
  3. Alternatively, check if your locality publishes inspection results online — many Virginia health departments do.
  4. Your inspection report shows each category as IN (compliant), OUT, or N/A. BOB requires all critical categories to be IN.
  5. Download your report as a PDF and upload it here.

Fire Marshal Certificate / Report
Current fire safety inspection or certificate from your local fire marshal or authority having jurisdiction (AHJ).

How to get your Fire Marshal Certificate:
  1. Contact your local fire marshal's office or city/county fire department.
  2. Request a mobile food vendor fire safety inspection for your truck or setup.
  3. They inspect your hood suppression system, fire extinguisher, propane, and fuel connections.
  4. Upon passing, they issue a certificate or signed inspection report. Upload that document here.
  5. If you operate in multiple jurisdictions, upload the most recent one.
